Health Screening

In the interest of student health and in promoting good health practices, Beavercreek City Schools conducts a program of health screenings. These screenings are conducted according to the guidelines of the Ohio Department of Health.  At Valley Elementary, these include:

• Vision Screening

• Hearing Screening

• Pre-Kindergarten Screening

   

Although not conducted routinely, other screenings occur as needed. These include:

• Pediculoses (head lice)

• Hypertension (blood pressure)

• Dental

• Height and Weight

Student vaccination records must be documented and provided by a healthcare practitioner. If your child is missing any of these required immunizations, contact your healthcare provider to get them before school starts. If your child has medical, religious, or philosophical exemptions, the necessary paperwork must be submitted before a student starts the preschool program.

Per the Ohio Revised Code 5104.014, Division B: Each child's caretaker/parent/guardian shall provide to the preschool a medical statement indicating that the child has been immunized against or is in the process of being immunized against all of the following diseases: Chickenpox (Varicella), Diphtheria (DTaP), Haemophilus influenzae type b (Hib), Hepatitis A, Hepatitis B, Influenza, Measles (MMR), Mumps (MMR), Pertussis (DTaP), Pneumococcal disease, Poliomyelitis (IPV), Rotavirus, Rubella (MMR), and Tetanus (DTaP).

Our medication policies are firmly enforced.

• a. Medication will be administered only from its original container which is from a prescription written by a licensed physician specifically for that child.
• b. A parent must provide a MEDICATION FORM filled out by a physician and handed directly to the child’s teacher or school nurse along with the medication.
• c. Non-prescription medication will not be administered unless accompanied by a note from a physician stating specific dosages for a specific child. This includes all over-the-counter remedies.
• d. When medication is administered, a written log, including date, dosage, and time, shall be made and kept on file.

The parent shall provide, prior to admission, and ANNUALLY from the date of examination thereafter, a physical form stating that the child is in suitable condition for enrollment in the program. Along with this information, proof shall be provided that the child has had all the required immunization for his/her age. The physical examination form MUST BE COMPLETED BY YOUR PHYSICIAN AND RETURNED TO YOUR CHILD’S TEACHER PRIOR TO STARTING SCHOOL. A dental examination signed by a licensed professional is also encouraged.

Special Medical Needs

When to Keep Your Child Home

Beavercreek Schools' policies with regard to sickness follow: 

• Fever:  If a child has a fever of 100.4 degrees or higher, they are to remain home until they are fever-free for 24 hours. (This does not mean fever-free after taking Tylenol or Motrin). 

• Vomiting:  Child may return to school after 24 hours from the last episode of vomiting.
